To Sign Out:
- Please fill-out this form: Request Form (Sign In W/ SBU Email)
- During pick-up, you will be asked to fill out a sign-out sheet for our records as well as a sign-in sheet when you return the items.
- Items may not be out longer than 48-hrs, or past two business days.
- Weekends do not count towards this time window as the office is not open on weekends.
- If items are lost or brought back broken, please refer to the RHA legislation for penalties: Legislation

***NOTE: FOR THE COTTON CANDY MACHINE, POPCORN MACHINE & SNOW CONE MACHINE, WE PROVIDE THE MACHINE/NECESSARY TOOLS, BUT NOT THE FOOD SUPPLIES NEEDED, THIS WILL NEED TO BE ALLOCATED FOR BY YOUR HALL COUNCIL.
EX: KERNELS, SEASONING, COTTON CANDY SUGAR, ETC.
